Pre-Enrollment Application

In order to complete your child’s admission process, you must submit the following information to BMI main office:

Medical/ Shot Record
Birth Certificate

These items may be delivered to the main office in person or by mail (certified mail is preferred). If any one of these items is not submitted, your child’s application for enrollment will not be considered.
